Tooth with hypercementosis. Tooth with irreversible pulp inflammation and hypercementosis. The visual cone test was compatible with the working length obtained with the aid of the apical locator, but the radiographic image apparently fell short of the radiographic apex. The length was reassessed with the apical locator and confirmed. The image of hypercementosis leads us to conclude that the foramen is actually below the false radiographic apex and that this is the correct length.
