Patient indicated for endodontic treatment of maxillary anterior teeth for aesthetic prosthetic rehabilitation. Historically, mandibular incisors have been positioned lingually for esthetic and restorative reasons. Anatomy of the mandibular incisors presents a challenge when making endodontic access due to their small size and high prevalence of two canals. Benjamin and Dowson (1) reported that the prevalence of two canals in mandibular incisors is 41.4%. With a lingual access, the lingual canal is often difficult to locate and instrument. When making a lingual access in mandibular incisors, an artificial protrusion of dentin remains making it difficult to detect and debride the lingual canal. Janik advocated extending the lingual access further toward the cingulum to aid in locating and debridement of the lingual canal. canal.

Others recommended a facial access to better locate and debride the anterior teeth. LaTurno and Zillich traced
straight-line access using 50 mandibular incisors. They found that 84% of these teeth would require access placed through the incisalfacial aspect to obtain straight-line access.
